Long-Term Equipment Loan Agreement
Disabled Student Programs & Services provides equipment for eligible students to use on a temporary checkout basis. Students must adhere to the following conditions:
Requests must be made at least 3 business days prior to the event. Requests submitted with less than 3 days’ notice may not be able to be granted. If the need arises to cancel the interpreter after a request is made, student must notify DSS/DSP&S/DRC (E-mail, VP, or landline) 48 hours in advance of the event.
- I agree to return the equipment in the same condition as when received
- I agree to pay for any corrective action that must be taken to restore or replace the piece(s) of equipment to their original condition
- I agree to not loan the equipment to any person
- I agree to return the equipment no later than the due date listed below
I further understand that if the equipment is not returned on time, or the damage/replacement fees are not paid, access to my RCC official records will be blocked until the fees are paid or the item(s) is returned in working order.
