Time Sheet: Verification of Hours Worked (Form D)

Credit for work experience is based on a certain number of hours worked. The Time Sheet verifies the actual number of hours worked each month to justify unit credit.

Students will complete the Time Sheet during the first week of each month to show the hours worked the previous month. For example, if a student is completing the form in June, they will enter the number of hours worked each week in May. If work begins at 8:00 a.m. and finishes at 5:00 p.m., students will compute the number of hours worked. Eight hours a day, five days a week, equals 40 hours.

DO NOT include lunchtime as hours worked.The same holds true for dinnertime and for those students who work afternoon and/or evening shifts. Holidays and vacation days ARE NOT to be included as hours worked.
DO NOT list hours worked as ""9 to 5" or "12 to 5", just list the amount of hours worked each week. It is required that the number of hours worked each month be totaled and placed in the space provided. At the end of the semester, the student AND THEIR SUPERVISOR must verify the hours worked and BOTH MUST SIGN the Time Sheet.
